This is a unique program crafted by Cypress physical therapists. Our “clinical eyes,” coupled with our years of experience, allow us to see early clinical changes and create a quick response. Here’s what we can do:

  • Help you or a loved one age in place by preventing hospitalization or further decline.
  • Be a bridge with family, guardians and physicians through strong communication.

We understand that family dynamics can make it challenging to address concerns for our parents, and sometimes these issues are better received from a health care professional. In this way, our therapists can be the middle ground, navigating those waters with dignity, integrity and compassion.

Our goal is communication — keeping you informed of how your loved one is doing from visit to visit. We can be the “front line” to see changes, concerns or decline — whether it be medication sitting idly in the pill box, a refrigerator with more expired items than not, weight loss, bruising from falls or signs of not drinking enough water. This is an Age in Place program at its best. We still address mobility, safety and strength activities during these visits, with a portion, per your direction, set aside to communicate with you regarding your loved one.
